The capsule management during hip arthroscopy continues to be an important concern independent of the approach (inside-out or outside-in). Preserving most of the anatomy of the capsule ligaments is challenging when we encounter hips with large cam deformities. T-capsulotomy is a good option to improve visualization, but it requires the disruption of the zona orbicularis, increasing the risk for developing instability if not repaired properly or hip adhesions and motion restriction when we do close it or secondary to the capsular disruption itself.
